What is an annuity?
-Individual Retirement Arrangement (IRA) and Flexible Premium Annuities are personal savings plans that provide tax advantages for setting aside money for retirement.

What is the current interest rate?
– Although interest rates fluctuate, the current rate is 2.25%.

How much can I contribute to my IRA this year?
-The most you can contribute to all of your Traditional and Roth IRAs is the smaller of:
• $5,500 (for 2014 and 2015), or $6,500 if you’re age 50 or older by the end of the year; or
• Your taxable compensation for the year.

What is my account balance?

Can I look up my account information on the website?

How do I have my annuity withdrawals directly deposited into my checking or savings account?
-Complete the Direct Deposit Agreement Form along with a VOIDED check or deposit slip and mail to:

How can I have my monthly premiums taken from my checking account?
-Complete the Request for Pre-Authorized Premium Payment Plan form along with a VOIDED check and mail to:

Premium payments must be current on all policies prior to set-up. If premiums are currently being paid through payroll deduction, the treasurer must be notified to remove the insurance from the local billing.

How can I take out a loan from my Life Insurance policy?
-Complete and sign the Loan Request form or write out a signed request and forward it to the UTUIA Policyholder Service Department.

Do I have to pay a life insurance policy loan back?
-Yes

How can I receive the dividends on my policy?
-Complete and sign the Dividend Request Form or write out a signed request and forward it to the Policyholder Service Department.

How do I change my beneficiary?
-Complete in detail the Change of Beneficiary Request Form. You must sign and return the original form for processing. A facsimile or photocopy will not be accepted. Be sure to sign your name as the owner and have a disinterested (someone other than a named beneficiary) party sign and witness.

The change will be updated on your insurance record, endorsed by the General Secretary and Treasurer, and a photocopy of this document will be mailed to you for your records.

I lost my policy. How can I get another one?
-Complete the Lost Policy Affidavit, sign and mail the form with original signatures to our Policyholder Services Department. A facsimile or photocopy will be not accepted.

Are claim forms available online?
-Yes, you can access the forms under the Forms and Resources tab.

Death Claims

What documentation in needed in order to file a death claim?
-A certified death certificate, a claim form completed by the beneficiary(ies) on record, the original life policy or completed Lost Policy Affidavit.

Can the original death certificate be returned?
-Yes, as long as there is a note requesting it to be returned.

What happens if the primary beneficiary is deceased and there is no contingent beneficiary named? What documentation is needed to file a claim?
-A copy of the death certificate for the primary beneficiary and letters of testamentary, a small estate affidavit or other fudiciary papers showing who is in charge of the insured’s estate.

There is a loan on the policy, how are benefits paid out at the time of death?
-The loan balance will be calculated as of the date of death and will be deducted from the proceeds paid to the beneficiary on record.

Can proceeds from the insurance policy be assigned?
-In most cases, yes. The listed beneficiary(ies) on the policy or the funeral home should contact the Claims Department at 1-800-558-8842 or email mmclucas@utuia.org.

What information is needed to file a death claim on a Cancer policy?
-If the insured was confined to a hospital overnight for the treatment of cancer, the beneficiary on record should submit a copy of the death certificate, our Cancer and Hospital Claim Form (CL1-D) completed by the beneficiary and the insured’s attending physician, a copy of a pathology report for the first diagnosis of the cancer and a copy of the hospital billing statement showing the admit and discharge dates.

Who else should be contacted regarding the death of the insured?
-The Railroad Retirement Board at 1-877-772-5772 and Metlife at 1-800-310-7770 (For Railroad Employees Only).

Disability Claims

When do I submit my first claim form for a Disability policy?
-In order to receive a full month of benefits, you need to submit your Disability Claim Form (CL-1) 30 days after your elimination period has ended. If the claim form is submitted sooner, your benefit will be pro-rated, and a partial month paid.

How often are disability claims paid?
-Claims are paid on a monthly basis (every 30 days).

How do I continually receive benefit payments?
-A Disability Claim Form is sent with each monthly payment and will indicate a date to return the form by. Upon receipt of the completed claim form, a monthly benefit check will be mailed.

How do I file a Group Disability claim?
-Anthem claim forms are available at www.utu.org or by calling 1-800-232-0113. UTUIA claim forms cannot be used for the Anthem Short Term Disability plan.  Anthem claims are not handled through the UTUIA.

Cancer Claims

How do I file a Cancer claim?
-A Cancer and Hospital Claim Form (CL-1H) must be completed by you and your doctor. You must also include a copy of the pathology report first diagnosing your cancer as well as a copy of the hospital billing statement showing admit and discharge dates. If you are unable to receive this, please provide the name of the hospital(s), city, state and dates that you were confined overnight. Claims can be submitted to:

Accident Claims

What documentation is required to submit a claim on my Accident Indemnity Policy?
-An Accident Indemnity Claim Form must be completed by you and your doctor. We also require any medical documentation supporting your accident in order to receive maximum benefits payable.

I would like to purchase a new Insurance policy. Who do I contact?
